That's the last survey for the cohort onboarding process. There's only one step remaining, which is creating an account with the Care Partner Connect, the community portal for the AKALAKA cohort. From there, you can chat with the cohort facilitator and other cohort members, find additional resources, post events and learn about events others are attending, and share the experience of being a Care Partner.
